Information Needed to Receive an Examination Copy of Second Edition of How to Succeed in College Mathematics


1.If the book is for a course: (a) course name, (b) course number, (c) course description, (d) expected date of offering, (e) expected enrollment, (f) how you heard about the book. Questions and comments are welcomed.

2.If the book is for a workshop: (a) description of the workshop content, (b) expected enrollment, (c) expected date of offering, (d) number and duration of sessions, (e) title(s) of workshop leader(s). Questions and comments are welcomed.

3. For items 1 and 2, provide: (a) adoption decision date, (b) who will make the decision, and (c) whether the likelihood of adoption is excellent, good, or slim.

4.Complete contact information, including (a) your title, (b) school name and address, (c) email address, and (d) telephone number. (Please provide an explanation if the course instructor’s or workshop leader’s name cannot be found on the institution’s website.)

5.If adopted, contact your bookstore and request that the book (and perhaps also the assignment manual) be ordered in the desired quantity from BergWay Publishing or from the bookstore’s vendor. If the book is positively reviewed but not adopted, please recommend it to your university library for purchase.

6.Examination copies will not be provided for courses or workshops with less than 6

   students.

7.Examination copies are not for resale.

8.An examination copy is sent at the discretion of the publisher; examination copies will only be mailed to your school, college, or university street address.

9.Requests for an examination copy must be received in writing on institutional letterhead. Requests must be mailed or faxed; they cannot be placed by telephone or email.

Requesting Desk Copy

A desk copy of the book is available to the instructor or workshop leader if at least six copies of the book have been ordered for the course or workshop, and he or she has not previously received a desk or examination copy. The same applies to the assignment manual.

Desk copies have to be requested; an examination copy becomes the desk copy if the book or assignment manual is adopted. Desk copies are not for resale.
